This is the second in a series of frequently
asked questions about the benefits provided by the DC 37 Health & Security
Plan. These questions focus on your vision benefits.
Q:
How can I get glasses? A: You and your eligible dependents can get
an eye exam, lenses and covered frames, every two years, dating from the last
time the full benefit (exam, lenses and frames) was used. If you used the full
benefit on, say, Aug. 1, 2007, you would be eligible again for the full benefit
on Aug. 1, 2009.
Q: How do I go about getting the optical benefit? A:
Call the Inquiry units forms line at 212-815-1531 to request an optical
voucher. This voucher can be used at any participating provider. A list
of providers will be enclosed with the voucher. You can also use the direct
reimbursement method, but you will only be reimbursed based on the plans
reimbursement schedule.
Q: What about the supplemental optical benefit? You
and your eligible dependents can get a supplemental benefit (eye exam and change
of lenses if necessary) on an annual basis, measured one year from the date the
full benefit was last used. For instance, if you used the full benefit on Aug.
1, 2007, you would be eligible for the supplemental benefit on Aug. 1, 2008. This
supplemental benefit is only available at the DC 37 Vision Center at 115 Chambers
